Your Portal to Personal Care

We're putting you in charge

At Valley Primary Care, we want to you to be engaged in your own well-being. That's why we offer a convenient tool to empower you to manage your healthcare.

If you've ever walked away from your healthcare provider’s office and forgotten what was said during the visit, or if you've thought of an important follow-up question hours later; then we've provided the patient portal with you in mind.

A patient portal is a website for your personal health care. The online tool helps you to keep track of your health care provider visits, test results, prescriptions, and so on. You can also securely e-mail your provider questions through the portal.

Making healthcare information easily accessible is a critical part of keeping you involved and informed in your care.  You can access your secure personal health information and be in touch with your provider's office 24 hours a day. You do not need to wait for office hours or returned phone calls to have basic issues resolved.

What can you do with the portal?

  • Request appointments (non-urgent)
  • Refill prescriptions
  • Ask questions through secure e-mail
  • View test results
  • If you have a child under age 18, you may be given access to your child's patient portal.

*Patient portals are not for urgent issues. If your need is time-sensitive, you should still call the Valley Primary Care office.

How do you use the patient portal?

Signing up for the portal is quick and easy. You will need a computer and internet connection to use it. Once you arrive at the web page, follow the instructions to register for an account. The service is free. A password is used so that all of your information is private and secure. Once you are in your patient portal, you can click the links to perform basic tasks. You can also communicate with Valley Primary Care in the message center.

At the time of your first office visit you’ll be asked if you would like to receive an email invitation to the portal. But you don't have to wait for an invitation. Follow this link to set up your account.

Valley Primary Care has a new location!

After three years of sharing the beautiful offices of A Healing Space, we are ready to move and grow into our next chapter. We look forward to continuing to provide everything you need for your health and well-being in our new clinic location.

Here are the important details:

  • 690 Van Buren Street, Eugene Oregon, 97402.
  • Monday, June 4, 2018 will be our first day in the new location. If you have an appointment on or after June 4th, come to our new clinic on Van Buren Street.
  • Office hours & phone number will remain unchanged: phone (541) 284-4043; fax (866) 428-6583; Monday – Thursday 8:30 – 5:00, closed Friday – Sunday.

The new location is ‘kitty-corner’ from the Shell station on 7th Street. You will find parking available on the street along with a designated handicapped parking spot adjacent to the building.
