You should never feel anxious or nervous about coming to visit our office. To help alleviate some of the anxiety you might feel about visiting Valley Primary Care for healthcare or counseling services in Eugene, OR, we have put together this FAQ to answer many of the questions or concerns you may have about our office and services.

What do I need to bring to my appointment?

Please bring your insurance card and photo identification as well as a list of medications you are taking, including any over-the-counter medicine, vitamins, or herbal medications. We also ask you to bring any forms that may need to be filled out by the medical provider for your work or school. Also any patient forms for new and existing patients.

What if I can’t make it to my scheduled appointment?

Please call the office to change your appointment as soon as possible. The Valley Primary Care policy is at least 24-hour cancellation notice.

What do I do if I have a medical emergency?

If you are experiencing a medical emergency, call 911 and seek medical care immediately. If you have an urgent medical need that is not life threatening after clinic hours please call (541) 284-4043 and you will be directed to the on-call clinician. If you wish to be seen immediately for an urgent need (after hours), you may choose to use the local urgent care facilities.

Is my medical information confidential?

Valley Primary Care strictly adheres to all healthcare privacy laws, and only the providers involved in your care or caretakers you designate will have access to your health information.

What health insurances do you accept?

Valley Primary Care accepts all major insurance plans.

When is payment due?

All payments are due at the time of service.
